To help us better understand you and your child, please prepare for your consultation by:
- Completing the Consultation Form (click the Consultation Form button below) to the best of your ability. If a question does not apply, write “N/A”.
- Sending a copy of this completed Consultation Form, with a copy of any assessments and education plans (IEP/504) you currently have. Please do not send your originals, and keep a copy for yourself.
- If you have not already done so, request a complete copy of your child’s school records.
Once we receive your Consultation Form and documents, we will review them and schedule your consultation. At the consultation we can discuss your needs, and whether we are able to represent you and your child’s interests. If we agree to begin an attorney-client relationship, a client agreement will be presented to you at that time. Feel free to contact us if you have any questions, or need assistance with this form.
